My approach to thyroid health starts with comprehensive testing not only including both free and bound thyroid hormone, but also antibody levels. Reverse T3 is often run, but this marker is greatly misunderstood and the commercially approved lab kits used for it's detection are not very sensitive or specific.
In addition to thyroid hormones, a neuroadrenal assessment is commonly recommended. We
look at serum DHEA-S and cortisol, conduct a 4-point salivary cortisol test, and urinary neurotransmitters; including epinephrine and norepinephrine which are mostly produced in the adrenal glands.
Stress and inflammation impact thyroid hormone activity on multiple fronts and therefore its presence must be assessed. If inflammation is present then addressing its root cause(s) will be an important part of a holistic thyroid management approach.
Many people experience a decrease in thyroid hormone activity in any chronic disease or condition that includes chronic immune activation or inflammation. Autoimmune activity is the a main cause behind hypothyroidism but often goes unmeasured and then not always factored into thyroid hormone dosing; typically your hormone needs increase directly to thyroid antibody activity. Unfortunately, people can have a subclinical hypothyroidism for years before lab work, including antibodies, can detect problems.
It is estimated that hypothyroid disease affects 10 million people and some people estimate that up to 10% of all females may have some form of hypothyroid disorder. This means there are a lot of people who's metabolic needs are not being met resulting in: persistent fatigue, lowered immunity, decreased metabolism, difficulty loosing weight, elevated cholesterol levels, insomnia, anxiety, depression, etc. A holistic, yet scientific approach to thyroid health is a better approach.
