Is Your Gut Messing with Your Hormones?

Updated: May 19

An overgrowth of bacteria in the small intestines, known as Small Intestinal Bacterial Overgrowth (SIBO), is typically discussed in the context of bloating, gas, and digestive discomfort. However, its impact extends far beyond the gut. One of the lesser-known consequences of SIBO is its ability to disrupt sex hormone balance, particularly by interfering with healthy estrogen metabolism. This disruption can contribute to symptoms such as PMS, heavy or irregular periods, low libido, mood fluctuations, and even infertility.

How SIBO Changes Estrogen Levels

There is a collection of bacteria in the gut, known as the estrobolome, that modulates the metabolism and recycling of estrogen in the body. When SIBO is present, the balance of the estrobolome is disrupted. This increases the activity of the chemical (beta-glucuronidase) which reactivates estrogen that was neutralized and tagged by the liver for elimination. Once reactivated, the estrogen goes back into circulation. The result is a build-up of recirculating estrogen, worsening symptoms of estrogen dominance, such as breast tenderness, menstrual irregularities, and fibroids.

How SIBO causes estrogen dominance and related symptoms

Why SIBO Makes it Harder for the Body to Clear Hormones

The liver plays a central role in hormone metabolism. It converts estradiol into less potent forms for removal. SIBO-related inflammation, toxin production (e.g., lipopolysaccharides or LPS), and nutrient malabsorption (particularly B vitamins, magnesium, and amino acids) can impair liver detoxification pathways. Impairment in liver detox makes it harder to clear hormones efficiently. This bottleneck further contributes to hormonal imbalances.

The Hidden Estrogen Imbalance Linked to Gut Issues

SIBO may also influence the balance of estrogen metabolites processed during liver detoxification. These metabolites are considered pro-inflammatory and potentially harmful. In fact, certain estrogen byproducts have been linked to DNA damage and estrogen-related cancers when not properly deactivated and excreted.

In individuals with SIBO, poor methylation status, gut microflora imbalance, and impaired detox pathways can favor the buildup of these estrogen metabolites. Patients with elevated levels of these byproducts often report symptom relief when using DIM (Diindolylmethane). Derived from cruciferous vegetables, DIM helps shift estrogen metabolism toward safer estrogen byproducts. DIM's efficacy in this context suggests a strong link between imbalances in gut microflora, impaired hormone detoxification, and estrogen-related imbalances.

SIBO’s Effect on Stress and Hormone Levels

Chronic inflammation from SIBO can activate the stress response, increasing levels of the stress hormone, cortisol. Elevated cortisol can suppress levels of the hormone (GnRH) responsible for stimulating production of estrogen, progesterone, and testosterone. This imbalance often presents as irregular menstrual cycles, low libido, anxiety, or fatigue, further complicating reproductive health.

The Role of Gut Motility and Hormonal Clearance

Efficient hormonal clearance requires regular bowel movements and healthy gut motility. When motility slows—as is common in certain types of SIBO—estrogen metabolites can linger in the intestines, increasing the chance of reabsorption. This reinforces a cycle of hormone recirculation and imbalance.

Final Thoughts: A Gut-Centered Approach to Hormone Balance

If you are experiencing unexplained hormonal symptoms alongside digestive issues, SIBO may be the missing link. Identifying and treating SIBO can restore gut microbiome diversity, reduce inflammation, support liver detox pathways, and optimize estrogen metabolism. All together, this can lead to more balanced hormones. A comprehensive approach that includes digestive support, hormone testing, gut healing protocols, and nutrient repletion can help address the root cause, improving both digestive and reproductive health.
